SERVICES:
Our physicians are fully trained to treat pediatric and adult patients with disorders of the skin, hair, and nails. In addition to the diagnosis and treatment of skin diseases and skin cancer, our services include:
• hair removal laser
• acne treatment
• psoriasis phototherapy including PUVA and Immunomodulators such as Enbrel,Amevive
• skin cancer surgery including Mohs micrographic surgery
• mole, cyst, & skin tag removal
• chemical peels
• LASER treatment of spider veins, angiomas, "age spots", freckles, and some birth marks.
• Restylane replacement therapy
• sclerotherapy for leg veins
• Botox Treatment for Wrinkle Reduction

OFFICE HOURS:
Monday -- Friday 8:30 am until 5:00 pm


YOUR FIRST APPOINTMENT:
Please arrive 10 minutes before your appointment so that you may complete our new patient information questionnaire. Please remember to bring any insurance information and a list of all medications that you take.

CANCELLING APPOINTMENTS:
Please call 24 hours in advance to cancel appointments so that we may offer that appointment time to other patients.

CALLING THE OFFICE:
Please call our office with any questions regarding your condition, medication, or treatment. Our staff is specially trained to help you with your questions and our doctors are always available.

FOR PRESCRIPTION REFILLS:
Please call or have your pharmacy call during our office hours when your medical record is available. An office visit may be required depending upon when you were last seen in the office or which medication you are refilling. This allows us to monitor for side effects and better determine whether the medication is still the best one for you.

BILLING AND INSURANCE:
Payment is requested at the time of service unless other arrangements have been made. We accept cash, check, Visa, MasterCard, American Express and Discover.

FOR INSURED PATIENTS:
Please be prepared to pay your insurance co-payment or deductible, if any. As a courtesy, we will prepare and submit your insurance claim for you though you will be responsible for any outstanding balance after 30 days.

FOR UNINSURED PATIENTS AND FOR NONCOVERED SERVICES (SUCH AS COSMETIC SERVICES):
Full payment is requested at the time of service though for larger medically necessary services a payment plan may be arranged.
